WHAT DO I NEED?
The following documentation is required by most insurance plans in order to determine if you qualify for insurance coverage:
1. A prescription from your physician containing the following:
- Detailed Description of the Item(s) Ordered
- Quantity Prescribed
- Length of Need
- ICD-10 Diagnosis Code
2. Chart Notes or Progress Notes provided by your physician that supports the medical necessity of the item(s) prescribed.
Your physician can fax the documentation outlined above to 314-725-1444.
You will need to call us at 314-725-1888, so that we can obtain your insurance information and set up your account.
We take care of the rest! We check your benefits, communicate with your physician to clarify your order and obtain prior authorizations that may be required by your insurance company. We keep you informed during the set up process and can deliver the supplies right to your door.
